The forms for Individual and Advanced Awards should be filled by the member and submitted to the Club President or VP Education who will sign and submit the form to the Area Director.
For Club Awards, the Club President or Vice President of Education should complete the form and submit it to the Area Director.
The forms may be digitally signed or wet-signed as long as they are submitted digitally (e.g. PDF or JPEG).
Completed forms may be submitted at any time.
Club Awards will be finalized after we confirm the April membership renewal.
